Heart Rehabilitation Clinic

Our program serves people who have been diagnosed with heart disease and need to reduce their heart risk factors. We give you and your family information, exercises and tools to help you manage your condition, improve your health and quality of life. 

Clients who take part in the program are less likely to return to the hospital and have a higher chance to improve their quality of life. 

You can access our program in person or virtually. 

Referral

You will need a doctor’s referral to attend the program. 

Get ready for your visit

Bring the following to your appointment:

• photo identification card
• healthcare card
• current list of medications

Wear clothing that is easy to move in and footwear that is appropriate for physical activity.
