Heart Function Clinic

Our program provides outpatient ongoing follow-up care, management and education for people living with heart failure. 

Heart failure happens when the heart cannot keep up with the workload of the body and needs lifelong support. 

Services

• initial and ongoing assessment
• individualized teaching
• ordering tests as needed
• referrals to other support services

Referral

You will need a doctor’s referral from the Grey Nuns Community Hospital to access our clinic.

Preparing for your visit

Please bring the following to your appointment:

• photo identification card
• healthcare card
• current list of medications

Monday - Thursday, 8 a.m. – 3 p.m.
